Sign the Authorization Card

To sign the card, complete the form and receive a PDF card via email or physical card in the mail to print, sign, and date. You can also simply download a PDF of the card and fill it out in its entirety. Please note, you MUST physically sign and date the card in order for it to be counted. We strongly recommend using a single pen with blue or black ink.

To return the card, you may either:

Scan or photograph your original signature and email it to card@uwresearchersunited.org; or
Mail your original signature to us at UAW 4121, 2633 Eastlake Ave E, Seattle WA 98102.
