Want a card physically mailed to you?

Fill out the form below, and we will mail you an authorization card. Please note, you MUST physically sign and date the card in order for it to be counted. We strongly recommend using a single pen with blue or black ink.

To return the card, you may either:

• Scan or photograph your original signature and email it to card@uwresearchersunited.org; or
• Mail your original signature to us at UAW 4121, 2633 Eastlake Ave E, Seattle WA 98102.
